3) Book your initial consultation

You can then book your first consultation with our clinical team to choose the most suitable equipment and introduce you to the therapy.

    Once you submit the form, we will email you requesting your prescription or letter. Simply reply with the PDF attached and one of our team can verify it and issue your Intus Plus contract for your review and signature.

    The Association for Respiratory Technology and Physiology (ARTP) Standards of Care state that written confirmation of CPAP suitability must be provided when purchasing a CPAP machine. We understand that providing this cannot always be done immediately, and the resulting delay could impact a patient’s health. We therefore ask that you accept the following declaration, to allow us to enroll you on Intus Plus and begin your therapy ahead of your providing that to us.

    I declare that I have previously been diagnosed with Obstructive Sleep Apnoea by a qualified medical professional.

    I do not currently have the written confirmation required but I will promptly request this from my GP or specialist, and forward it to you within 28 days.

    Therefore, in the interim, I accept full responsibility for making the choice to begin CPAP therapy and will not hold Intus Healthcare Ltd liable in any way
    whatsoever.

    I understand that this declaration is used as a temporary measure only and I will still need to provide written documentation to Intus Healthcare.

    If you’re not able to commit to this declaration, we would recommend choosing the “No, I have not been formally diagnosed” option above instead. We can then send you a £50 discount code for our In-Home Sleep Test, reducing it from £189 to £139. This will give you a formal diagnosis within days and we can then fast-track your Intus Plus registration.

    Without a formal diagnosis, we are unable to enroll you on Intus Plus. However with our In-Home Sleep Test you could get that diagnosis within days. Ordinarily £189, we can email you you a £50 discount code. Once you have your diagnosis, we can fast-track your Intus Plus registration.
